Mothership's Volunteer Application form is available here as an .odg file (LibreOffice). These are the bases for the forms linked above. We made some changes to the files. For example, we are not currently able to provide court-mandated community-service hours as far as I know. For this and other reasons we can't just use Mothership's forms directly.
Our forms aren't perfect, but they are public. If you need to make a change to them, please get a login to the atrium site and post your changes there.
